回 旋枝 (LCx) へ の 血 行再 建 が key とな る症 例 に お い て, #11へCABGを 施 行 した 一 例 を報 告 す る. 症例 は74歳 の 女性. 冠 動脈 造影 検 査 に て#2100%, #7100%, #990%, #1190%の 三枝 病変 を認 め た. OMお よびPLは hypoplastic で あ り吻 合 は 困難 で あ る と思 わ れ た. 左 室 駆 出率 は31% と低 値 で あ っ た. Tlシ ンチ グ ラ フ ィー で は前 壁 の 一 部 お よび後 側 壁 に viability を認 め た. 手 術 は #11にSVGを 単 独 で, #8・#9にSVGを sequential に吻 合 した. #11の 露 出 に は若 干 の 工夫 を要 した. 血 管 径 は大 き く確 実 な吻 合 を行 えた. 本 方法 は, LCx末 梢病 変 が な く#11中 枢 側 に 高度 な病 変 を認 め る場 合 や, OM, PLが と もに run-off 不 良 な ため 吻合 に適 さな い場合 に有 用 な方 法 で あ る と考 え られ た. 日心 外会 誌27巻5号: 310-313 (1998)
