Amaç: Helicobacter pylori duodenal ülser dışında kronik atrofik gastrit ve intestinal metaplaziden de sorumlu olduğu düşünülmeye başlandı. Bu üç lezyonun bir arada bulunma sıklığı ve H. pylori ile olan ilişkilerini araştırmak çalışmamızın temelini oluşturmuştur. Yöntemler: Bu nedenle H. pylori (+) pozitif duodenal ülser tanılı ve nonülser dispepsi yakınması olan hastalarda atrofik gastrit ve intestinal metaplazi sıklığının H. pylori (-) negatif olan aynı grup hastalarla karşılaştırılması amacıyla veriler toplanmaya başlanmıştır. Çalışmamızın amacı H. pylori (+) pozitiliğinin atrofik gastrit ve intestinal metaplazi gelişme sıklığı üzerine etkisinin araştırılmasıdır. Bulunan sonuçlar H. pylori (-) negatif ve (+) pozitif olan duodenal ülser, nonülser dispepsili hastalarda değerlendirildi ve H. pylori (+) pozitifliğinin atrofik gastrit ve intestinal metaplazi etiyopatogenezindeki yeri istatistik anlamlık açısından tartışılmıştır. Bulgular: Çalışmamızda da literatür ile uyumlu olarak duodenal ülser ve antral gastrit birlikteliği H. pylori (+) pozitif olan grupta ve 40-59 yaş arasında daha sık görüldü. H. pylori (-) negatif olan grupta daha sık görülen antral gastritin yaşlara göre dağılımından H. pylori (-) negatif olan grup ve H. pylori (+) pozitif olan grup arasında herhangi bir anlamlı fark saptanmaz iken her iki grupta da antral gastrite en çok 40-59 yaş arasında rastlandı. İntestinal metaplazi H. pylori (-) negatif grupta daha sık görülürken, yaşlara göre dağılımında ise H. pylori (-) negatif olan grup ve H. pylori (+) pozitif olan grup arasında anlamlı fark yoktu. Gruplar kontrol grubu ile karşılaştırıldığında anlamlı fark bulundu. İntestinal metaplazi yaş gruplarına göre dağılımında gruplar arasında anlamlı fark saptanmadı. Her üç grupta da intestinal metaplazinin 40-59 yaş arasında daha sık olduğu görüldü. Sonuç: H. pylori duodenal ülser, atrofik gastrit, intestinal metaplazi gelişmi üzerine etkili bulunmuştur. The frequency of coexistence of these three lesions and their relationship with H. pylori was the basis of our study. Methods: Therefore, data were collected to compare the incidence of atrophic gastritis and intestinal metaplasia with the same group of patients with H. pylori (+) positive duodenal ulcer and non-ulcer dyspepsia. The aim of this study was to investigate the effect of H. pylori (+) positivity on the incidence of atrophic gastritis and intestinal metaplasia. Positivity of atrophic gastritis and intestinal metaplasia was discussed in terms of statistical significance. Results: In our study H. pylori (+) positive group and 40-59 years of age, it was more frequent in the group of H. pylori (-) negative. There was no significant difference between the two groups in the group with positive antral gastritis between 40 and 59 years. There was no difference between the control group and the control group. Intestinal metaplasia was not detected according to the age groups. Conclusion: H. pylori was found to be effective on the development of duodenal ulcer, atrophic gastritis, intestinal metaplasia.
