フ レネル膜 プ リズ ム(以 下膜 プ リズ ム)の 貼 り間違 いに よる例 を示 す。 13歳 の女子 、先 天 内斜視 に対 して膜 プ リズ ムで眼位 と両眼 視機 能 の経過 観察 中、 前 回 に処 方 した 膜 プ リズム を装用 して受診 した際、 内斜 視の増 大、 両眼視 機 能 の低 下 が生 じた。 しか し、担 当 した 視 能訓練 士(ORT)は これ らの原 因 を追 究す る ことがで きなか っ た。再検 査 に よ り、膜 プ リズ ムの 貼 り間違 いが生 じて い るこ とが判 明 した。視 能矯正 中の ヒヤ リハ ッ ト防止 策 と して次 の点 をあ げる。 1. 先 入観 、思 い込 み を しない 2. 自覚 的 な装 用感 を確 認す る 3. 膜 プ リズ ム装用 に違和 感 等が あれ ば連絡 をとる 4. 膜 プ リズ ムの装 着 は基底 方 向 を確 認す る 5. 眼鏡 チ ェ ックは担 当 したORTが 行 う 6. カル テ にその経 過 を記載 す る
