交代性上斜位(Dissociated vertical deviation以下DVDと略す)の頭位異常は、麻痺性斜視にみられるような常に一定方向への頭位異常ではなく、その種類も様々である。そこで私たちは、このような頭位の動きがDVDにみられる眼位の動揺や、両眼視機能に関係しているのではないかと考え、明らかな頭位異常を示したDVD14例を対象に、頭位異常と眼位および両眼視機能を検討した。症例の頭位異常はHead tilt、Face turn、Chin upもしくはdownと様々であったが、全例に両眼視機能は認められず、症例の示した頭位異常は、両眼視獲得目的ではないことが分かった。また、眼位の動揺に伴って不規則な頭位異常を示す者が14例中11例(79%)に認められ、それら11例中9例(82%)は頭位異常のない時に比べ頭位異常を示した時の方が非固視眼の上転が大きくなる傾向を認めた。したがってDVDの頭位異常は、両眼開放下で単眼視が誘発された時に固視眼を最も見やすい位置にするために生じるのではないかと考えられた。
