Some remarks on the examination and treatment of anisometropia are discussed. Visual acuity in amblyopic eyes is usually examined under monocular conditions. However, since amblyopia results from disturbances in binocular interaction at the cortical level, measurement of visual acuity under binocular conditions may be of importance. The Binocular Sepa(R) and SGtest with an occlusion foil bar were employed to measure visual acuity of anisometropic amblyopic eyes under binocular conditions, and to evaluate the suppression from the sound eye. Seven out of 8 cases had a decrease in visual acuity in the amblyopic eye when examined with the Binocular Sepa(R) under binocular conditions. Four out of 5 cases who had visual acuity better than 0.6 under binocular conditions had a good stereopsis better than 50 arcsec at near distances. Occlusion foil therapy was performed following total occlusion in 17 anisometropic amblyopia patients of which 13 (76%) were school-aged children, and it appeared to be useful to maintain their binocularities. Careful followup is needed, as regression of visual acuity may take place after cessation of the treatment by the age of 12 years, a critical period for anisometropic amblyopia. Furthermore, the psychological state of school-aged children during treatment needs to be taken into account.
