【Purpose】We reported a patient with residual refractive errors corrected by Laser in situ keratomileusis (LASIK) in one eye and Femtosecond laser astigmatism keratotomy (FsAK) in the other eye after multifocal intraocular lens (IOL) implantation. 【Case】The patient was a 67-year-old male who previously underwent bilateral cataract surgery with diffractive multifocal IOLs and was referred to our clinic for correction of residual refractive errors. Preoperative distance visual acuity (DVA) was 0.3 (1.0 × -1.00 D cyl -1.00 D Ax170°) in the right eye and 0.4 (1.0 × +0.25 D cyl -1.50 D Ax30°) in the left eye. Uncorrected near visual acuity (NVA) and distance-corrected NVA (corrected DVA) were 0.4 and 0.9 in the right eye, and 0.4 and 0.8 in the left eye, respectively. Wavefront-guided LASIK was performed in order to correct myopic astigmatism in the right eye. Corneal oblique astigmatism (1.40 D Ax124°) in the left eye was detected by corneal topography (TMS4, TOMEY) and FsAK, a minimally invasive procedure that avoids damages to the optical portion and corneal epithelium, was performed. To prevent overcorrection, the operations were performed with a wide optical zone and a narrow incision. Postoperative DVA at 20 months was 0.8 (1.2 × +0.25 D) in the right eye, 0.6 (1.2 × -0.75 D cy1 -0.50 D Ax30°) in the left eye, and 1.0 in both eyes. NVA was 0.6 (0.9 × +0.75 D) in the right eye, 0.7 (0.9×±0.00 D cyl -0.75 D Ax30°) in the left eye, and 0.9 in both eyes. Corneal astigmatism in the left eye was reduced to 0.67 D. 【Conclusion】Additional corrections by LASIK are effective when residual refractive errors occur after cataract surgery especially with multifocal intraocular lens implantation and good uncorrected visual acuity is not obtained. FsAK with a higher degree of precision to correct simple astigmatism will be a future topic.
