要 約 【目的】術前屈折値(PR)および水晶体厚(LT)の入力の有無がHolladay 2式に与える影響を検討 すること。 【対象と方法】対象は白内障術前にIOLMasterモデル500(Carl Zeiss Meditec、以下IOLマス ター)と超音波Aモード法で生体計測を行い、眼内レンズを完全嚢内固定後、合併症がなく、術後 矯正視力0.5以上の181名277眼。角膜屈折力40 Dと48 D、眼軸長(AL)22 mmと26 mmを境に 対象を9群に分類し、前眼部の大きさとALの分布を調べた。またPR、LT(Aモード測定値)の有 無により、IOLマスターの入力方法をA法: PR有LT有、B法: PR無LT有、C法: PR有LT無、D 法: PR無LT無に分け、Holladay 2 式の術後屈折誤差(=術後2ヶ月以降の自覚屈折値-予測値) を比較した。 【結果】対象の88 %は前眼部とALの均整がとれていた。AL 22 mm 未満を含む277 眼ではPRを入 力すると有意に近視化した(p<0.05、paired t-test)。AL 22 mm 未満を除く257 眼ではA・Bおよ びC・D法間のみ有意差を認め(p<0.05)、PRを入力すると近視化したが、A・C法間、B・D法間 に有意差はなかった。 【結論】Holladay 2 式はPRを入力すると近視化する。LT入力の有無は術後屈折誤差に影響しない。
