【Purpose】To study the effect of accommodation intervention, we used cycloplegics when measuring refraction with an internal fixation autorefractometer and a binocular open-field autorefractometer.To investigate the difference between the two devices, objective measurements of refraction before and after cycloplegia were obtained and compared to subjective measurements of refraction. 【Subjects and Methods】Subjects were 306 eyes of 153 participants who had no ophthalmic diseases except refractive error of -3.00D(cylinder) or less. Subjective refraction tests were performed and objective measurements by aninternal fixation refractor and abinocular openfieldrefractor before and after cycloplegia using Mydrin-P eyedrops (Phenylephrine hydrochloride/tropicamide) were obtained. During the measurement by the binocular openfieldrefractor, a mirror was set two meters away from the binocular open-fieldrefractor and the subject fixated in the center of the two eyes. 【Results】The measurements after cycloplegia by the binocular open-fieldrefractor were the closest to the subjective refraction. As compared to the measurements before cycloplegia, the measurements after cycloplegia by the internal fixation refractor and the binocular openfieldrefractor were significantly more hyperopic with average differences of -0.46±0.46D and -0.27±0.34D, respectively (P<0.01). As compared to the measurements after cycloplegia by the binocular open-fieldrefractor, the measurements after cycloplegia by the internal fixation refractor were significantly more myopic for an average difference of -0.45±0.29D(P<0.01).The measurements before cycloplegia by the internal fixation refractor were the most myopic measurements and were significantly more myopic than the measurements after cycloplegia by the binocular open-fieldrefractor for an average difference of -0.91 ± 0.54D (P<0.01). 【Conclusion】The measurements after cycloplegia by the internal fixation refractor were significantly more myopic than the same measurements by the binocular open-fieldrefractor.This suggested that with the internal fixation refractor, the accommodation intervention could not be inhibited completely even with the use of Phenylephrine hydrochloride/tropicamide. On the other hand, the binocular open-fieldrefractor could prevent a myopic tendency in its measurements at a significant level and the measurements before/after cycloplegia did not very much showing less accommodation intervention.
