痔瘻 ・肛 門周 囲膿 瘍 の長期 経過 中 に認め られ た著 明 な肛 門狭 窄 に対 し,自 動縫 合器 に よるcircularstaplingが 有 効 であ った興 味 あ る1例 を経験 したの で報告 す る。症 例 は41歳 男性。17歳 か ら27歳 まで に痔核 に対 し1回,痔 瘻 に対 し2回,痔 核 ・痔瘻 に対 し1回 の手術 を受 け た。 その後40歳 までに肛 門周 囲膿 瘍に対 し,十 数 回の切 開 ・ 排膿 を受 け た。40歳 頃 よ り排 便 困難 が出現 。他 院 で肛 門狭 窄 と診 断 され,緩 下 剤 の投 与 を受 け たが軽 快せ ず,当 科 を紹介 され た。入 院後,ピ ンホー ル大 の肛 門狭 窄 に対 し,S状 結 腸双 孔式 人工肛 門 を造設 した。その2カ 月後 に 人工 肛 門 よ り大腸 内視 鏡下 にanvil headを 狭 窄部 直上 まで誘導 し,経 肛 門的 に外経33mmのcircular staplerを 挿 入 し,anvilheadと 連 結 させ,狭 窄部 を打 ち抜 いた。3カ 月後 に人工 肛 門 を閉鎖 した。 人工 肛 門閉鎖 後3年10カ 月の現在,示 指 頭大 の 内腔 を保 ち,排 便 障害 は認 め て いな い。
