胃癌 術後,十 二指 腸 断端 の縫合 不全 に対 し,経 皮 的に フ ィブ リン糊 を充填 し短 期 間に瘻 孔 を閉鎖 で きた症例 を 経 験 した ので若 干 の文献 的考 察 を加 え報告 す る。 症例 は65歳,男 性。 残 胃癌,肝 転移 の診 断 で,残 胃全 摘,Roux-en Y再 建,肝 外側 区切 除術 施行 。第5病 日に Winslow孔 ドレー ンよ り胆 汁様 の浸 出液 を認 め,腹 部CT検 査 で膵 前面 に膿瘍 腔 と思 われ る低 吸収域 を認 め た。ド レー ンの持 続吸 引 お よび抗生 剤 を使用 し,徐 々 に炎症所 見 の改善 を認め た。瘻孔 造影 に て十二 指腸 断端 の縫 合不 全 と診断 し持 続 洗浄 を開 始 した。細 く長 い瘻 孔が 形成 され たが完 全 閉鎖 に は至 らず,第56病 日に経 皮的 に フ ィブ リン糊 充填 を行 った。瘻 孔 の閉鎖 を確 認 し第64病 日退院 とな った。 感染 の ない径 の細 い瘻 孔 の形成 が フ ィブ リン糊充 填 に よる瘻 孔 閉鎖 を促 し,十 二指 腸縫 合不 全治癒 につ なが っ た と考 え られ る。
