人工 心肺 に伴 う好 中球 活性 化 に よる術後 呼吸 障害 に対 す る,白 血球 除去 フ ィル ター(LG-6)の 有用性 を検 討 した。検 討Aで は成 人待機 手術 連続34例 をLG-6使 用群 と通 常の フ ィル ター使用 群 に分割 し比較 した。好 中球数 や 過酸化 脂質 の推 移 では両 群 間 に有 意差 は見 られなか ったが,Respiratory Index(AaDO2/PaO2)は 術 後6時 間値 が有 意にLG-6使 用群 で 良好 であ った。検 討Bで は緊 急症例 や小 児例 を含 めLG-6を 使 用連 続185例 を,術 後3時 間 で人工呼 吸 を継続 して いた171例 を術 後 呼吸 障害 の リス ク群(n=81)と 非 リス ク群(n=90)に 二分 し,急 性 期 の呼吸 状態 を比較 した。FiO2は リス ク群 で有意 に高 か った が,PaO2お よびP/Fratio(PaO2/FiO2)は 両群 間に 有 意差 は認 め なか った。LG-6使 用 に よ りリス ク群 にお いて も術 後 急性期 の呼 吸機 能 は非 リス ク群 と同様 に保 ち うるこ とが 示唆 され た。
