キーワード:腎性副甲状腺機能亢進症,副甲状腺摘出,血液透析,シナカルセト,手術予測因子 〈要旨〉 腎性副甲状腺機能亢進症の治療において,内科的治療抵抗性の場合,副甲状腺摘出術(parathyroidectomy:PTx) などが考慮される.PTx件数はシナカルセト承認後に減少しているが,未だに PTxが必要な症例が存在している. また,予後に対する影響,費用対効果,手術成績などの要因もあり,ガイドラインどおり一元的に PTxが行われて いるわけではないのが現状である.今回,シナカルセト内服症例において,PTx施行に至った症例と保存的治療継 続中の症例を比較検討することで,PTxに至る手術予測因子を模索した.PTx施行群において,シナカルセト内服 1年目の血清 iPTH 308.05 pg/mLがカットオフ値の候補としてあがった.今回算出された値が,PTx施行時期を考 慮するうえで参考になる可能性があると考えられた.
