キーワード:長期透析,粘液管状紡錘細胞癌 〈要旨〉 長期透析患者に発生した粘液管状紡錘細胞癌の 1例を経験したので報告する.患者は 65歳男性,1999年に糖尿病 性腎症で血液透析導入.2013年 4月から肉眼的血尿が出現し 10月の定期 CTにて左腎腫瘍を指摘され当科紹介. CTでは左腎臓に 65 mmの腫瘤を認め,傍大動脈リンパ節腫大も認めた.左腎細胞癌(cT1bN2M0)と診断し根治 的左腎摘除術およびリンパ節郭清術を施行.肉眼的に腫瘍の境界は明瞭で割面は黄褐色調であった.組織学的に異 型性の顕著な腫瘍細胞が管状~乳頭状構造を呈して浸潤性に増殖しており紡錘形細胞も混在していた.免疫組織学 的には CK7(+),CK34βE12(-),vimentin(+),CD10(+),P504S(+),TFE3(-),CA9(-)であり,Alcian blue染色で間質が陽性であった.以上から粘液管状紡錘細胞癌と診断した.現在術後 6か月経過するが再発なく生 存中である.
