SARSは38度以上の発熱で発症し病初期にはインフルエンザと鑑別困難である. SARSとインフルエンザとの鑑別に有用な知見を得る目的で, 2002年11月から2003年3月期における血液透析患者に対するインフルエンザワクチン接種回数別の抗体価, 臨床症状を分析した.対象は, 単一医院の外来維持血液透析患者80例 (平均年齢60.1±12.0歳, 平均透析歴6.9±5.4年, 男性46例, 女性34例, 糖尿病22例, 非糖尿病58例) とした. これらの中でワクチン未接種群は16例, 1回接種群7例, 2回接種群57例であった. ワクチン接種時期は, 2002年10月下旬と11月下旬, 抗体価測定は翌年1月上旬, 臨床症状の観察は同年2-3月に行った.臨床的有効抗体価 (HI法40倍以上) の達成率は, 未接種群に比較して2回接種群で, 有意に高く, さらに1回接種法より2回接種法が高い傾向を示した. 臨床症状は, インフルエンザワクチン接種群 (1回+2回接種群) 64例中18例 (28.1%) に感冒症状の出現を認めたものの, その中で38度以上の発熱を認めたのはわずかに1例 (1.6%) で, 2回接種群においては皆無であった.SARS流行に備えて, インフルエンザワクチン接種の徹底は, 透析施設の現場を混乱させないために必須のものと考えられた.
