目的: シャント不全の早期診断における超音波Bモード・パルスドップラー複合法の有用性を検討した. 対象と方法: 対象は慢性血液透析患者53名で, 超音波Bモード・パルスドップラー複合法によりシャント側上腕動脈の血流量と抵抗指数およびシャント静脈の最小血管内径を計測した. 血液透析時の最大脱血速度が150mL/min未満のものをシャント不全と診断した. 結果: シャント不全例はシャント発育不全5例と狭窄への経皮的血管形成術を施行した3例であった. 上腕動脈血流量と抵抗指数の間 (r=-0.65), 最小血管内径と上腕動脈血流量 (r=0.60) または抵抗指数 (r=-0.45) との間に有意の相関を認めた, ROC (receiver-operating-characteristics) 曲線よりシャント不全の境界値を検討したところ, 上腕動脈血流量は600mL/min, 抵抗指数は0.67であり, 感度は各々88%と100%, 特異性は98%と96%で精度はいずれも96%であった. 最小血管内径では感度と特異性の良好な境界値は設定できなかった. 結語: 超音波検査によるシャント不全の早期診断にはシャント側上腕動脈の血流量または抵抗指数が境界値付近の症例について超音波断層像に加えてシャント造影等による血管内径の検討を行うのがよいと考えられた.
