心臓カテーテル操作後に腎不全となり, 腎生検および皮膚生検によりコレステロール塞栓症と診断した1例を経験したので報告する. 症例は57歳男性, 不安定狭心症の診断で, 冠動脈造影 (CAG) および経皮経管的冠動脈形成術 (PTCA) 施行後, 非乏尿性の腎機能低下を呈し, 血液透析に導入となった. 透析導入時の腎生検, およびカテーテル操作後に出現した足趾チアノーゼ部の皮膚生検で, ともに血管内腔にコレステロール塞栓が認められた. カテーテル操作とその後の過剰な抗凝固療法が原因と考えられた. その後, 維持透析を行っていたが, 次第に腎機能の改善を認め, 導入後約1年2か月で透析を離脱した. コレステロール塞栓症には特発性および二次性があり, 本症の確定診断には, 生検による組織診断が必要であるが, 急性あるいは慢性腎不全の原因として念頭におくべき病態と考えられる.
