key words: ブ ラ ッ ドア クセス, 上腕 動脈 表在 化, 創 〓開, 人工 真皮, 維 持 透析  症例 は50歳 の 女性 であ り, 妊 娠 中 毒症 によ る慢 性 腎不 全 に よ り血 液 透析 に導入 され そ の後20年 間 維 持 透析 を 行 ってい る. 内 シ ャン トに よ り血液 透析 を行 ってい たが, シ ャン ト不全 によ り血 流 が とれ な くな ったため, 左上 腕動 脈 表在化 手術 を受 けた. 術 後2週 間 目に抜 糸 した ところ創の 〓開 が起 こった. 保 存 的 に治 療 してい たが治癒 傾 向が現 れ なか っ たた め手術 的 に治療 を行 った. 〓開 創 をdebridementし たの ち, 人工真 皮 (テル モ株 式会社 製 テル ダー ミス真 皮 欠損用 グラ フ ト) を移 植 した. 人工真 皮移 植後2週 間 目 に良性 肉芽様 組織 によ り変 換 され た. その時 点 で, 鼠径部 よ り採皮 し植 皮 した ところ良 好 な皮膚 生着 が得 られ た. その結果, 表 在化 した上腕 動脈 は現 在blood accessと して良好 に機 能 してい る.
