disopyramide (DSP) 投与により副作用の発現と, 時に効果不十分で不整脈をおこすCAPD (continuous amburatory peritoneal dialysis) 施行中の1症例を経験した, 当初外来にてDSP 100mg分1を投与されていたが, コントロール不良のため入院し, 200mg分2に増量された. 増量後に視覚異常が現われたため減量したが視覚異常は続いた. また, 眼のかすみからくる嘔気から食欲不振となったため, この時点で血中濃度のモニタリングを開始した. 測定結果より最高血中濃度 (2.87μg/ml) が高いため副作用が発現し, 最低血中濃度 (0.75μg/ml) が低すぎるため不整脈が発現したものと考え, DSPの血中濃度の変動を少なくするためDSP 100mg分2の投与に変更した. 変更後, DSPの最高血中濃度は2.52μg/ml, 最低血中濃度は1.31μg/mlとなり, MND濃度は1.24-1.58μg/mlと一貫して高値であったが副作用なくコントロールできるようになった.
