International audienceThe objective of this study consist in displaying sociotechnical implementations which strongly intricate mucoviscidosis, a rare disease, within the medical field. Following the traces of cystic fibrosis, we are showing the central character of the accomplished standardizations by the social-technological network which surrounds it, in order to ensure its integration within the field of medicine. The major changes are concern­ing the medical perception, due to knowledge consortium, common medical practices and professional network. The clinical approach is still founded on medical practices, especially regarding chronical diseases. And, thus, Socio-technical network changes the pathological entity of Cystic fibrosis into a matter on which physicians can apply a clinical professional ap­proach. Four major steps need to be enlightened. The first stage deals with the constitution of the cystic fibrosis as a distinct clinical entity defined and shared on an international level. The second stage involves the centralization of these new patients within a small area where a detailed col­lection and assembly of information begins. By the third stage, cystic fibrosis is no longer seen as an incurable disease as medical discoveries increase and medical assistance is based more and more on local protocols. The Cystic fibrosis resource and competence centres make up the final stage, creating a technical structure and autonomous organization, bringing together the practical inventories and solving conflict between actors. These four stages make up a standardiza­tion process of cystic fibrosis within the world of medicine. In this study, the standardization around cystic fibrosis looks to link a rare dis­ease with the clinical reasoning which guides the standardization of care, of their organization and their quality. Once accomplished, it favors the institutionalization of the disease within the medical world.L’objectif de cette recherche consiste à mettre en évidence les dispositifs sociotechniques impliqués dans l’intégration progressive d’une maladie rare dans le champ médical. En suivant les traces de la mucoviscidose, il s’agit d’établir le caractère central des standardisations accom­plies par le réseau sociotechnique qui la façonne et lui assure un mode d’existence à chaque fois spécifique. Les transformations majeures de la qualification de la mucoviscidose sont à mettre en rapport avec la standardisation des savoirs, des pratiques et des relations tissées au sein d’un réseau professionnel. Ce travail permet la transformation d’une entité pathologique initiale­ment confuse en une maladie rare répondant aux exigences d’une démarche clinique encore incon­tournable pour les maladies chroniques. Quatre étapes de transformation dans les mécanismes de standardisation sont à distinguer. La première étape aboutit à la constitution de la mucovisci­dose en tant qu’entité clinique distincte, définie et partagée au niveau international. La deuxième étape marque la centralisation de ces malades reconnus comme porteurs de la mucoviscidose dans un petit nombre de lieux où commence un fin travail de récolte et d’assemblage d’informations. La troisième étape fait sortir la mucoviscidose de l’impuissance médicale : les découvertes se multiplient et les prises en charge se structurent sur la base de protocoles locaux qui essaiment. Les Centres de Ressources et de Compétences de la Mucoviscidose constituent la dernière étape, réalisant un dispositif technique et organisation­nel autonome, intégrant à la fois l’inventaire des pratiques et la résolution des tensions entre les acteurs. Ces quatre étapes construisent un proces­sus de standardisation et d’institutionnalisation de la mucoviscidose dans l’espace médical. La démarche clinique guide ce processus
