In some cases of allogeneic bone marrow transplantation (BMT), enough amount of bone marrow mononuclear cells cannot be obtained because of hypocellular marrow or anemic status of donors. For the purpose of getting sufficient volume of bone marrow safely, a pilot study to collect increased volume of donor's bone marrow with a support of once-a-week subcutaneous administration of recombinant human erythropoietin (rHuEPO) was performed.Six allogeneic bone marrow donors received rHuEPO subcutaneously on pre-BMT days 14 and 7, and predonated 400ml of blood on the same days to total volume of 800ml. Each donor received oral iron preparations for 21 days. The hemoglobin level just before BMT was not significantly decreased against preadministration level (mean: -0.3g/dl).Median volume of 19.5ml (range: 15.2-27.4ml) per kilogram of donor's body weight was collected from donors with the decrease of hemoglobin level of 2.0g/dl compared with preadministration levels. At least 3.0×108 nucleated cells per kilogram of receipient body weight could be obtained from all donors. None of donors showed any adverse effects.This protocol is considered to be very practical and safe to obtain enough volume of donor's bone marrow cells for allogeneic BMT.
