To investigate the proportion of hepatitis B infectious blood that can be excluded by the HBc antibody screening test, we measured the HBc and HBs antibodies from three HBs antigen positive donor groups, group 1: RPHA over 25, group 2: RPHA under 24 but positive, group 3: RPHA-negative but EIA-positive.Most blood from each donor group showed an HBc antibody titer over the 26 cut-off level (group 1: 98%, group 2: 88%, group 3: 61%), and HBs antibody titer below 22 (group 1: 99%, group 2: 92%, group 3: 79%). However, some donors showed low or no titer of HBc antibody, or high titer of HBs antibody. The number of these donors seemed to increase in inverse proportion to the HBs antigen titer. In case of blood donors carrying both HBs antibody and quite low HBs antigen, the subtype of HBs antibody tended to be anti-r.Therefore, we conclude that a slightly more strict cut-off level and HBs subtype be considered in the HBc-HBs screening test.
