鼠径(部)ヘルニアの分類において,一般的に外鼠径ヘルニア=間接ヘルニア,内鼠 径ヘルニア=直接ヘルニアと認識されている.しかし,本来ヘルニア門の位置(内,外) と逸脱経路(直接,間接)は別の概念の分類様式である.今回われわれは下腹壁動静脈 外側に内鼠径輪とは別のヘルニア門を持ち,かつ横筋筋膜の破壊を伴って逸脱する鼠径 部ヘルニアの 1例を経験したので報告する.症例は75歳の男性.左下腹部痛にて当院を 紹介受診.CT等にて左鼠径ヘルニア嵌頓として緊急手術を行った.鏡視下に確認する と下腹壁動脈の外側にヘルニア門を認め,小腸のRichter型嵌頓を認めた.嵌頓をきた したヘルニアとは別に内鼠径輪をヘルニア門とするde novo 型I型のヘルニアの合併 も認め,併せて鏡視下にポリプロピレンメッシュにて修復を行った.本症例は日本ヘル ニア学会の分類によるとV型(分類不能)と分類されるが,病態としては「直接型の外 鼠径ヘルニア」とも呼べるまれな疾患であると考えられた.
