本研究は,山口大学医学部附属病院医薬品等治験・ 臨床研究等審査委員会で審査を受け,承認された。 対象患者は2006年9月から2009年8月までに山口 大学病院高度救命救急センターに緊急入院し横紋筋融 解症と診断された患者のうち,本人または家族の同意 が得られ,検体採取が可能であった患者である。横紋 筋融解症の診断は,原因(外傷,圧迫,虚血,感染, 代謝障害など)の存在,CK値の上昇(1,000 IU/l以上) および血中ミオグロビン値の上昇(1,000 ng/ml以上) で行った。入院時(day 0),1日後(day 1),3日後(day 3),7日後(day 7)の随時尿を採取し,尿中L-FABPを enzyme-linked immunosorbent assay(ELISA)法で測 定した。血中ミオグロビン値はCardiac Reader System(ロシュ・ダイアグノスティックス)を用いて 測定した。一般的な腎機能検査に加え,ICU在室日数 や血液浄化療法の導入の有無について調べた。急性腎 不全の診断は,1血清クレアチニン(creatinine, Cr) 値≧2.0 mg/dl,2血清Crの増加率≧0.5 mg/dl/day, または3乏尿(≦400 ml/day)のいずれかを満たす場 合とした。
