脳脊髄液減少症は,脳脊髄液腔から脳脊髄液 (髄液)が持続的ないし断続的に漏出することに よって脳脊髄液が減少し,頭痛,頸部痛,めまい, 耳鳴り,視機能障害,倦怠などさまざまな症状を 呈する疾患である。本疾患は症状が多彩であり, 定義にもみられるように,めまいは脳脊髄液減少 症によくみられる症状の一つであり,耳鼻咽喉科 を受診する例もあると考えられる。 今回,軽微な外傷後,めまいを主訴に耳鼻咽喉 科を受診し,異常眼球運動より脳脊髄液減少症を 疑い,画像検査により脳脊髄液減少症の診断に至 った2例を経験したので,若干の考察を加えて報 告する。 症 例 [症例1]27歳,男性 主訴:めまい,ふらつき 既往歴:特記事項なし 現病歴:201X年10月20日 テント造設中に鉄 パイプが,前頭部やや左側へと当たり挫創を形成 し,その後より頸部~腰部正中に鈍痛,左上肢の しびれが出現した。10月27日 回転性めまいを自 覚した。めまいは座位にて増悪し,30分~数時間 程度続くものであった。蝸牛症状はみられなかっ た。11月5日 近医脳外科にて頭部造影MRI施 行されたが異常は認められなかった。11月29日 Cerebrospinal fluid leakage is associated with several symptoms, such as headache, nausea, photophobia, diplopia and lassitude. In our practice, dizziness is a frequent presenting symptom in patients with this disorder. However, the mechanism and features of dizziness are unclear. In this report, we examined two patients with cerebrospinal fluid leakage with dizziness, exhibiting in particular flutter-like oscillations. Ocular flutter is characterized by intermittent bursts of horizontal saccades lacking an intersaccadic interval. Although the precise mechanisms are unknown, ocular flutter is often observed in cerebellar or brainstem lesions. In patients such as ours, ocular flutter may indicates that a central nervous system disorder may be present in cerebrospinal fluid leakage with dizziness. 原 著
