Surgical antibiotic prophylaxis (SAP) entails the prevention of infectious complications by administering an appropriate antimicrobial agent prior to exposure to contamination during surgery. However, a delicate balance exists between causing and preventing harm. In the setting of SAP, the risk:benefit ratio revolves around balancing the risk of adverse drug reactions and microbial resistance against the benefit of reduced incidence of surgical site infection (SSI). Therefore, optimising SAP may contribute towards cost saving benefits, improved patient outcomes and responsible antibiotic stewardship.
