Přestavba genu ALK (anaplastická lymfomová kináza) patří u nemalobuněčného karcinomu plic (NSCLC) ke genetickým změnám, které nám predikují účinnost cílené léčby ALK inhibitory. V současné době máme k dispozici již tři generace ALK inhibitorů. Alektinib, současný standard prvoliniové léčby ALK pozitivního NSCLC, je vysoce selektivní a potentní inhibitor ALK a RET tyrosinkinázy a od 1. 11. 2019 má i v České republice stanovenu úhradu v 1. linii léčby ALK+ NSCLC (1, 2).
