Severe oral ulceration caused by the use of methotrexate is one of the adverse effects that medication can cause. Low-level laser therapy has been used for ulcer therapy since it accelerates tissue repair, reduces edema and pain, and has an effective anti-inflammatory action with stimulation of fibroblast proliferation, tissue regeneration, immune response and neovascularization. The prevalence of involvement of oral ulcerations occurs in 11% to 17% of patients treated with MTX and generally health professionals have difficulties in associating these oral manifestations as a side effect of the medication, causing delays and difficulties in treatment. In this context, the article aims to report a clinical case of diagnosis and treatment of oral ulceration caused by the use of methotrexate by stopping the medication and using laser therapy as an adjunctive therapy. It was possible to obtain resolution of the clinical picture, with no recurrences or pain complaints after the treatment.
