To the Editor: In their excellent article,Molecular Approaches to the Identification ofUnculturable Infectious Agents, Gao and Moore(1) point out that molecular approaches should beunleashed on diseases such as sarcoidosis, Kawa-saki disease, and type I diabetes mellitus, whichare thought but not proven to be infectious. Theauthors, however, are overlooking the more com-mon and most likely infectious disease of unknownetiology today—prostatitis.According to the pathologist McNeal, theprostate gland is the most commonly diseasedinternal organ of the human body (2). Prostatitisis the most common prostate disease, resulting inmore physician visits than either benign pros-tatic hyperplasia or prostate cancer, according tothe National Institutes of Health (3). Despite itsfrequency, prostatitis as a disease and as a histo-logic lesion is understudied (4).By the Meares and Stamey culture localizationprocedure, in which the first voided urine, a mid-stream urine, the expressed prostatic secretions,and a final voided urine are compared, more than90% of cases in patients with chronic pelvic symp-toms are labeled as “nonbacterial” prostatitis orprostatodynia, both of which are thought to beincurable diseases (5).The University of Washington has documentedwhite blood cell counts as high as 38,000 per mm
