現病歴:登山中に転倒, 5 mの高さから落下し胸部を打 撲.当院に救急搬送された. 入院時身体所見:体温 36.3°C,血圧 182/137mmHg,心拍 数 108回/分 整, SpO 91% (マスク 10L/分).顔面蒼白, 頻呼吸,頚部から前胸部にかけて皮下に握雪感を認めた. 入院時検査所見:白血球が 23,200/mmと上昇, AST 96IU/l,ALT74IU/l,LDH459IU/l,CK297IU/lと,組織 挫滅による変化を認めた. 10L酸素下で pOは 77mmHg であった. CT:両側の気胸,縦隔気腫,著明な皮下気腫,左右の多 発肋骨骨折,右鎖骨骨折を認めた.気管中央から分岐部 付近にかけての膜様部における裂傷が疑われ,日本外傷 学会気管・気管支損傷分類 2008の Ib(T~C,mem)と診 断した (図 1). 手術所見:鈍的外力による気管損傷と診断し,気道確保 及び気道減圧のため気管切開,また左胸腔ドレナージを 施行した.皮下気腫が強く,気管切開の際は逸脱予防の ためカニューレを両側鎖骨頭に直接縫合固定した (図
