18 F-FDG PET scans have proven to be useful in the diagnosis and management of encephalitis patients. 18 F-FDG PET scans are also the standard of care in the evaluation of epilepsy patients before surgery. Encephalitis patients who later develop epilepsy may have useful imaging findings at the time of diagnosis. We present a case of 18 F-FDG PET imaging in a patient with recurrent cryptogenic meningoencephalitis. 18 F-FDG PET imaging after resolution of the encephalitis revealed hypometabolism in previously hypermetabolic areas. Hence, the initial 18 F-FDG PET scan prospectively predicted the epileptogenic zone and seizure-onset zone.
