要 旨:IPMN MCN 国際診療ガイドライン 2012 が 2006 年ガイドライン初版を改訂する 形で発表された.与えられたテーマである“IPMN の手術適応”を考える時,IPMN の型分 類が基本となるが,従来,施設間で型分類の混乱が認められた.今回,MD―IPMN,BD― IPMN,混合型 IPMN の定義が初めて示され,定義の混乱が解消されることが期待される. また,新たな分枝型 IPMN の診療のアルゴリズムも示された.アルゴリズムでは,診療を 考える起点となる“high―risk stigmata”に修正が加えられ,さらに“worrisome features” が新たに加えられた.こうした因子を中心に経過観察,手術適応を考えることとなった. 手術適応は概ね,現在,我が国で行われている適応に近いと思われるが,2~3cm の BD― IPMN や>3cm の BD―IPMN では若い患者さんの場合,手術を考慮しても良い,もしくは, 強く手術を考慮するとの記載があり,我が国の現状とはやや異なっていると考えられる. ガイドライン 2012 の解説とその問題点について概説した.
