目的探讨同期经尿道等离子电切治疗前列腺增生症(benign prostate hyperplasiaBPH) 合并浅表膀胱肿瘤(superficial bladder tumorSBT)的安全性和有效性。方法根据膀胱肿瘤的位置、大小、数目和前列腺大小、腺体与包膜粘连情况采用等离子电切方法同期治疗86例BPH合并SBT术后给予膀胱灌注化疗。结果86例手术顺利。PKRBt手术时间(42.2±25.3)minPKRP时间手术时间(72.3±23.2)min。术中闭孔神经反射9例膀胱穿孔3例无电切综合征、肠穿孔未发生死亡病例。术后病检均为前列腺增生膀胱尿路上皮癌5例出现短期尿失禁经盆底功能锻炼后好转。3例尿道狭窄经定期尿道扩张后治愈。术后6个月国际前列腺症状评分(IPSS)为(7.4±4.3)分、最大尿流率(Q)为(18.2±4.5)ml/s与术前(27.8±4.2)分、(5.4±3.3)ml/s相比明显改善(P0.01P0.001)。86例随访1~6年12例膀胱肿瘤复发复发部位均无尿道及前列腺窝种植转移。结论同期经尿道等离子电切治疗前列腺增生及浅表膀胱肿瘤安全、可行、疗效可靠。具有手术时间短创伤小、术后恢复快等优点。
