近年提唱されている胸腰椎外傷分類のほとんどは,脊 柱不安定性の評価として後方靭帯組織(posterior ligament complex:PLC)(Fig. 1 a)を重視したものとなっ ている.Magnetic resonance imaging(MRI)は PLC損 傷の評価で最も重要な放射線学的検査であり,T2強調 画像(T2WI),short tau inversion recovery(STIR)画像 で正確に high intensity areaとして描出される(Fig. 1 b, c).しかし,胸腰椎外傷は高エネルギー外傷に伴うこと が多く,ほかの全身外傷を合併していることも少なくな い.全身状態やバイタルサインが不安定な症例では,外 傷の救急治療においてMRIを撮影するのは容易ではな い.救急の現場において撮影が比較的容易な computed tomography(CT)で PLC損傷の有無を評価できれば, 早期に治療方針を立てるのに非常に有用である.今回わ れわれは CTを用いて,PLC損傷を診断可能か後方視的 に調査した.
