1 .脊椎高位と脊髄高位 この両者の関係は成長とともに大きくずれていくが成 人では一定の対応関係を示す.筆者はかつて剖検時に約 100 体から頚椎を en bloc で採取し脊髄神経根の脊髄起 始部への広がりから髄節高位を求め,椎体との位置関係 を調べた.そして多くの神経学書と照合したところ麻酔 科学の教科書の中には筆者の知見と合うものも散見され たが,これまでよく使われていた神経学教科書のほとん どと一致をみなかった.もっともこれは 1970 年代のこ とで現在では必ずしもあてはまらない.当時一番筆者の 知見と正しく一致したものは 1893 年版の Sir William Richard Gowers『A Manual of Diseases of The Nervous System』(1886)に示されている図であった(Fig. 1). これは驚きであり自分の目で見て確かめたものが一番正 しいことを知った.Gowers の図以前のものには,今日 的に使用にたえるものは見出せず,Gowers のこの図は 一番古くしてしかも現在にも貴重な解剖図譜といえるも ので以来ずっと愛用している.頚椎では脊髄高位は椎体 と 1.5 節の違い,椎体 T11 中央で第 1 腰髄がはじまり, 椎体 T12/L1 間に広がる脊髄 L4-S2 を円錐上部,椎体 L1 後方の脊髄は円錐部といわれ脊髄は腰椎 L1/2 間で 終糸となる.
