Corneal intraepithelial neoplasia is a dysplastic condition within the ocular surface squamous cell neoplasia spectrum. It is a rare condition that is seldom suspected, especially at its diffuse presentation. Once suspected, diagnosis and treatment are easy for a condition that if left untreated could be devastating. Clinical case: We present the case of a 63-year-old male patient presenting with a history of two weeks of right eye visual acuity decrease as only symptom. Right eye visual acuity was 20/40 and physical examination revealed a right cornea upper half geographic lesion with frosted appearance. After diagnostic rule out, epithelial scraping was decided, with low-grade corneal intraepithelial neoplasia reported by the pathology department. Management with mitomycin C 0.02% in two 2-week cycles with 3 topical applications per day, accompanied by fluorometholone 0.1% at the same dosage was then decided.
