癒合歯は下顎前歯部に多いと報告されている。今回、上顎右側第大臼歯と第大臼歯の癒合歯の症例を報告する。その癒合歯の組織構造、化学組成と発生過程を検索する目的で本研究を行った。26歳の男性の症例であり、上顎右側第大臼歯と第大臼歯の癒合歯である。第大臼歯の歯冠部の形態は正常であり、その歯根部で癒合している第大臼歯の歯冠はエナメル滴様の形態であった。第大臼歯の歯髄腔内にエナメル質が観察された。エナメル小柱の配列は不規則で、その横断形は鱗状であった。X線分析顕微鏡像では歯髄腔内のエナメル質は正常なエナメル質より透過性が高く、Pの濃度分布が低かった。球間象牙質が顕著に観察された。第大臼歯と第大臼歯の境界部では脈管象牙質が観察された。第大臼歯の歯冠形成の初期の段階で第大臼歯と癒合したと考察される。
