━━ Background. Paraneoplastic neurological syndrome (PNS) is mostly associated with small cell lung cancer and rarely associated with squamous cell lung cancer. We experienced a rare case of PNS with squamous cell lung cancer. Case. A 60-year-old male visited the neurological department in January 2011 due to numbness in the hands and feet. Since he was found to have lung cancer, he was suspected of having PNS and subsequently referred to our hospital for chemotherapy and a neurological evaluation. Histopathological studies revealed squamous cell lung cancer. Although the size of the tumor decreased after two courses of chemotherapy with docetaxel and carboplatin, the patient developed pneumonia before the third course of chemotherapy. Since the neurological symptoms worsened, as the tumor was growing, and an anti-Hu antibody test turned positive, he was treated with seven courses of plasmapheresis. Despite this therapy, the neurological symptoms did not improve, and he died due to progression of lung cancer. An autopsy revealed demyelination of the posterior funiculus and neuronal loss in the dorsal horn of the spinal cord. Conclusions. Although rare, squamous cell lung cancer may cause PNS. In cases with a possibility of PNS, it is necessary to diagnose the disease precisely and start anticancer treatment and immunotherapy as soon as possible. Laboratory Data on Admission 【Hematology】 【Biochemistry】 TP 6.9 g/dl WBC 15520/μl T-Bil 0.6 mg/dl Alb 3.10 mg/dl Neutro 92.50% AST 17 U/l CRP 22.81 mg/dl Lymph 5.00% ALT 24 U/l Mono 2.00% ALP 180 U/l 【Tumor markers】 Eosino 0.00% γ-GTP 13 U/l SCC 8.4 ng/ml Baso 0.50% LDH 125 U/l CYFRA 4.3 ng/ml CPK 18 U/l CEA 9.8 ng/ml TC 123 mg/dl ProGRP 20.5 pg/ml Na 130 mEq/l NSE 30.2 ng/ml K 44 mEq/l Cl 96 mEq/l 【Antineuronal antibodies】 Ca 8.2 mEq/l Anti-Hu (+) BUN 16 mEq/l Anti-Yo (-) Cr 0.5 mg/dl 症 例 症例:60 歳.男性. 主訴:発熱. 既往歴:C型慢性肝炎. 生活歴:喫煙歴は 1日 60 本を 40 年間.飲酒歴はない. 現病歴:2011 年 1 月中旬より両下肢のしびれが出現 し,近医神経内科を受診した.感覚優位の末梢神経障害 に併存して,胸部X線および computed tomography (CT)検査で両肺野の腫瘤影と肺門部および縦隔リンパ 節の腫大を認め,肺癌による傍腫瘍神経症候群が疑われ, 精査加療目的に当院へ紹介受診となった.気管支鏡検査 は,右上葉入口部の擦過細胞診および経気管支肺生検 (transbronchial lung biopsy;TBLB)で扁平上皮癌の診 断に至った.CT検査で右肺門部に縦隔リンパ節と一塊 になった腫瘍を認め,左肺の舌区にも転移と思われる腫 瘍を認め,両側肺門部と縦隔リンパ節の腫大も認めた. 右胸水も少量認めた.その他の胸腹部に明らかな転移は 認めなかった.骨シンチグラフィーでは右第 4肋骨に集 積を認め,右肺の腫瘍の直接浸潤と考えられた.頭部 MRI では脳転移は認めなかった.以上より,cT4N3M1a stage IV と診断し,ドセタキセル(docetaxel;DOC)110 mg(70 mg m2)+カ ル ボ プ ラ チ ン(carboplatin; CBDCA)590 mg(AUC 5)の化学療法を 2クール施行し, minor response(MR)となった.しかし,3クール直前 に発熱と食思不振から当院を受診し,肺炎を合併してい たため入院加療となった. 入院時現症:体温 38.5°C,血圧 98 72 mmHg,脈拍 108 分,SpO2 89%(室内気),意識清明,頚部リンパ節 触知せず,心音純,両側下肺野に湿性ラ音(+),下腿浮 腫(-),脳神経 II~XII に異常なし,徒手筋力テスト: 両側三角筋・上腕二頭筋・上腕三頭筋で 5 5,両側手首 以下で 4 5,両側大臀筋・腸腰筋・大腿四頭筋・ハムス トリングスで 4 5,両側腓腹筋・前脛骨筋で 3 5,深部腱 反射は四肢で消失,深部覚および温痛覚が遠位部優位に 低下していた. 入院時検査所見(Table 1):末梢血白血球の上昇,CRP 高値,正球性正色素性貧血を認めた. 入院時胸部X線所見(Figure 1A):右上肺野に均一に 透過性の低下を認め,左中肺野に径 35 mmの腫瘤陰影を 認めた. 胸部CT(Figure 1B,1C):右肺門部にリンパ節と一塊 になった腫瘍を認め,それにより右上葉は閉塞性の無気 肺となっていた.その他,左肺舌区に径約 40 mmの腫瘍 と縦隔のリンパ節腫大,右胸水を少量認めた. 入院後経過:肺炎に対してパズフロキサシン 1000 mg 日で点滴加療を開始したところ,第 6病日には解熱 し炎症反応も改善したため,第 15 病日にDOC 110 mg+ CBDCA 590 mgの化学療法を第 3クール目として施行 した.また,傍腫瘍神経症候群に特異度の高い抗Hu抗体 を測定したところ,定性検査で陽性であった.易感染性 の状態でありステロイドの適応は低いと判断したため末 梢神経障害の治療として血漿交換療法を試みた.第 28 病日より週 2回のペースで計 7回の二重膜濾過法による 血漿交換療法(血漿分離器:OP-08W,血漿分画器:EC30W,置換液:FFP)を施行した.経時的な神経障害の 変化を評価するために握力の測定を行ってみたが,血漿 交換療法期間中も明らかな改善は認めなかった.化学療 法後の胸部X線で腫瘍は縮小傾向であったが,その後の 血漿交換療法前後で施行した誘発筋電図検査では感覚神 経障害が進行しており,スプーンの把持が困難になるな ど,症状は徐々に増悪した.また,抗Hu抗体も血漿交換 Paraneoplastic Neurological Syndrome with Squamous Cell Lung Cancer―Kanzaki et al 38 Japanese Journal of Lung Cancer―Vol 55, No 1, Feb 20, 2015―www.haigan.gr.jp Figure 1. A mass of approximately 40 mm in diameter was detected in the lingual lymph nodes, causing swelling in the mediastinum and right pleural effusion (C). 療法の前後で陽性のまま変化はなかった.その後,胸部 X線で左胸部の腫瘤影が再度増大し始め,肺炎を合併し 第 71 病日に喀血を認め,同日永眠された.ご家族の承諾 を得て,病理解剖を行った. 左肺上葉の肺癌は S4b が主座であり,径 6.5×5 cm,胸 膜浸潤と左右肺門部へのリンパ節転移を認めた(Figure 2).組織学的には腫瘍細胞は小型から中等大であり,異 型性が強く,粗剛淡明な核小体を有するものを認めた. また,多くは充実性配列をとっていた.免疫化学染色で chromogranin A と synaptophysin がごく一部の胞体に わずかに染まり,thyroid transcription factor-1 染色は陰 性であった.左肺上葉の腫瘍は扁平上皮癌小細胞型と診 断した. 右肺上葉の肺癌は S2胸膜直下にあり径 4×7 cmの腫 瘤を形成し,胸膜外にまで浸潤し,第四肋骨と気管下部 にも浸潤を認めた(Figure 3).S2原発の腫瘍は組織学的 に大型な好酸性胞体をもつ細胞が重層配列をとり,細胞 間橋も明らかであり,癌真珠の形成を認めた.Chromogranin A,synaptophysin,thyroid transcription factor1 染色はすべて陰性であった.以上より扁平上皮癌分化 型と診断した.なお,両側の肺門部,気管分岐部,気管 周囲,上縦隔,頚部,胃幽門部周囲,膵頭部体部周囲に までリンパ節転移を認めた.リンパ節には扁平上皮癌小 細胞型と分化型の両方の転移巣を認めた.これらの所見 から,左肺と右肺の腫瘍は重複して発生した肺癌と判定 された. 腰髄では(Figure 4),luxol fast blue(LFB)染色で後 索の神経線維の高度の脱髄を認め,また,neurofilament (NF)免疫染色では軸索の消失を認めた.灰白質の神経細 胞には異常は認めなかった.
