背景Gefitinibは再発非小細胞肺癌に対して18.4%の奏効率を有するが増悪PD後一旦休薬したのちの再投与リチャレンジに関しての報告は少ない対象·方法2002∼2008年の6年間に当施設で再発非小細胞肺癌にGefitinibを投与し奏効後不応となり全身抗癌化学療法後にPDで再度Gefitinibを2回以上投与した24例を対象としPS·腫瘍マーカー·抗腫瘍効果をretrospectiveに検討した結果女性/男性が21例/3例54∼84歳中央値62歳全例が非喫煙者腺癌IIIB∼IV期であった初回∼5回目投与はPRが12例/3例/0例/0例/0例SDが11例/18例/8例/3例/1例PDが0例/3例/1例/2例/0例であったPSの改善/不変/悪化は12.5∼29.2%/40.0∼87.5%/0%であった腫瘍マーカーは初回と2回目は改善傾向で3回目以降は悪化傾向であった結論Gefitinib 1∼5回目の再投与について検討した結果2回目までの投与が最も有用であることが示唆され今後のprospectiveな研究に期待したいまた実地臨床においてリチャレンジの適応タイミングが今後の検討課題となると考えられた
