目的. 肺がんの新しい治療法としての胸腔鏡下肺葉切除・リンパ節郭清の成績を検討した. 対象と方法. 1992年9月から2004年8月までに378例の胸腔鏡下肺葉切除を行った. うち, 肺がんは324例であり, 重篤な合併症のない臨床病期I期肺がん247例で開胸術と同等のリンパ節郭清を行った. 成績. 切除したリンパ節は573 (平均25) 個であった. 組織学的検索の結果, N0が159例 (64.4%), N1が32例 (13.0%), N2が56例 (22.6%) であり, 最終的な術後病期はI期が152例であった. I期152例のうち, 5例はがん死したが, ほかは1130カ月再発の兆候なく生存中である. I期非小細胞がんの5年生存率は94.4%, 10年生存率87.9%であり, 以前に開胸術を行った症例の予後 (78.5%, 65.0%) に比べ明らかに良好であった (p=0.0013). 術前後の呼吸機能検査の成績は, 肺活量, 1秒量ともに胸腔鏡手術の方が開胸術に比べ低下が軽微であり, 術後3年までのフォローアップの結果, 胸腔鏡手術群では肺活量は術前値を1とした時に0.92まで, 1秒量は0.88まで回復した. 結語. 胸腔鏡下肺葉切除・リンパ節郭清はI期肺がんの標準術式の1つとして考えられる.
