目的. 皮膚筋炎の悪性腫瘍合併率は高く, 特に肺癌の合併が近年多数報告されている. そこで当院における皮膚筋炎合併肺癌の手術および剖検例を臨床病理学的に検討した. 対象. 1997年1月から2003年5月までの肺癌手術313例中の皮膚筋炎合併2例 (0.6%) と, 1976年から2003年までに剖検した皮膚筋炎症例12例中, 肺癌を合併した2例 (16.7%). 結果. 肺癌手術2例はともに2003年に経験した. 2例ともに男性で年齢は68歳と78歳であった. 病理組織所見はそれぞれ, 中低分化型扁平上皮癌と中分化型腺癌を認めた. 剖検12例中6例に悪性腫瘍を合併し, その6例中肺癌合併は2例 (33%) で1995年と2002年に経験した. 2例ともに男性で年齢は77歳と65歳であった. 病理組織所見はそれぞれ, 大細胞癌と小細胞癌を認めた. 結論. 皮膚筋炎に合併した肺癌の組織像はすべて異なり一・定の傾向はなかった. 全例高齢男性で, 近年になり肺癌合併例が多くみられた. 今後, 高齢男性の皮膚筋炎症例を経験した場合は, 消化器癌以外に肺癌合併にも十分注意し精査する必要がある.
