小型肺腺癌の特色と予後因子を明らかにするために, 腫瘍最大径2cm以下の末梢部肺腺癌92例を検討した.また, surfactant apoprotein A (SPA), urine protein 1 (UP1) について免疫組織化学染色を行い検討した.腫瘍最大径1cm以下 (5年生存率100%), 腫瘍最大径1.1-1.5cm (同89.7%), 高分化 (同96.3%), 細気管支肺胞型 (同100%), 細胞丈「低」(同96.0%), n0 (同88.3%), p0+p1 (同86.3%), ly (-) (同93.6%), v (-) (同93.7%) が有意に予後良好であった.SPA強陽性例は5年生存率86.1%であったが, n2が3例見られ早期症例に限定されなかった.多変量解析ではly因子, n因子, 腫瘍最大径が有意な予後因子であった.腫瘍最大径1cm以下, 細気管支肺胞細胞型, 低細胞丈の症例は全例リンパ節転移及び肺内転移を認めず, 以上の条件が整えば, リンパ節郭清の省略・縮小や縮小手術の適応の可能性が示唆された.
