原発性肺癌術後再発は, 治療方法が確立されておらず, その予後は不良である. 1986年から1993年の8年間に当科で切除した原発性肺癌309例のうち, 術後リンパ節再発に対して再手術を施行した8例について検討した. 初回手術時のリンパ節郭清は, 後側方開胸によるR2a郭清が5例, R2b郭清が1例で, 胸骨正中切開によるR3γ郭清が2例であった. リンパ節再発部位は, 頸部リンパ節4例, 縦隔リンパ節1例, 腋窩リンパ節2例, 頸部及び縦隔リンパ節1例であった. 初回手術からリンパ節再発の再手術までの平均期間は1年4ヵ月であった.現在生存中の2例を除き, 再手術後の平均生存期間は1年4ヵ月で, 再手術によっても治癒が得られなかった. 全症例を検討すると (血行性リンパ節転移と考えられる1例を除き), 初回手術時の徹底的な系統的リンパ節郭清の必要性と, p-N 3γ症例の治療の困難さが挙げられた.
