要 旨:症 例 は52歳 男性.普 段 か ら咳 と痰 が で るが4ヵ 月前 の胸部X線 写真 では異常 陰影 は なか った.正 月頃 か ら症 状 が増悪 した為 人間 ドッ ク入院 とな った.胸 部X線 写真 で左 下肺 野 に辺縁 やや 不鮮 明 の8mm大 の陰 影 が認め られ,胸 部CTで は左S5の 胸 膜直下 の 円形 の腫 瘤 を認め た.胸 膜 陥入像,spiculationは 認め られなか った.血 沈 の充 進,軽 度 の貧 血 は あ ったが,白 血球 の増 多は な く,痰 の培養 でH.haemolyticus+~++,抗 酸菌 陰性,細 胞診classII~ma,気 管支 肺洗 浄液 でclassIIで あ った.重 喫煙者 で もあ り,気 管 支 炎治 療 後 も陰影 は不 変 で,肺 癌 も否 定 し きれ ない為 開胸術 をお こな った.結 果 は炭粉 沈着 の 著 明な肺 内 リンパ 節 であ った.報 告例 は少 な く本邦 報告例 を ま とめ た.成 人病検 査,人 間 ドッ クな どで遭 遇す る機会 が あ り,鑑 別 診 断の1つ に肺 内 リンパ節 を考慮 す る必要 が あ り報告 した. 〔肺癌33(6): 953~957, 1993〕
