消化管吻合の創傷治癒過程に関しては,基 礎 的研究では病理組織学的検討1),膠 原線維や粘 液多糖類などの生化学的検討2),臨 床的には吻 合法,縫 合材料3)な どの比較検討がなされてい る。最近では,吻 合部微小血管造影所見の検討4) も行われ,さ らに組織酸素分圧5),組 織血流量6) などの循環動態の検討が報告されている。 術後合併症である縫合不全の原因としては, 全身的因子および局所的因子が考 えられる。局 所的因子 としては吻合部の血流障害が重要 と考 え7),手 縫い吻合 と器械吻合 についての組織血 流量を経時的に測定 し,縫 合不全の発生につい て検討,あ わせて病理組織像および耐圧試験 を 行い,両 者の耐圧点について も比較検討 した。 近年器械吻合の発達がめざましく,食 道癌, 大腸癌,食 道静脈瘤などの消化器手術における 器械吻合の有用性が認識 されてきている。 それ らのうち臨床的な報告では,縫 合不全について は器械吻合においては非常 に少な く,し かも発 生 してもminor leakageで あることが特徴的 日本大学医学部第3外 科学教室 別刷請求:〒101東 京都千代田区神田駿河台1-8-13 駿河台日大病院外科 河口忠彦 投稿受付:1990年3月22日
