近年,脳動脈瘤に対する血管内コイル塞栓術(CE)の普 及により相対的に直達手術によるネッククリッピング (NC)の施行率は減少傾向にある.しかし血管内治療を 専門とする術者は依然として不足しており,第一線の脳神 経外科の大部分ではNCが第一選択として行われているの が現状である.しかもマンパワーの問題から経験症例数 の少ない若手脳神経外科医が直達手術を担うことも少なく ない.一方,治療成績に対する社会的要求はますます高く なっており,直達手術を行う若手脳神経外科医には少ない 経験にもかかわらず良好な結果が求められている.若手脳 神経外科医の手術教育に関する報告は散見されるが ,若手術者の視点に立った手術成績に関する報告は少な い. 今回専門医取得前に,第一助手として約 60 症例の NC の経験ののち,専門医取得後に著者が指導医のもとで術者 として行った破裂動脈瘤に対するNCの手術成績を検討し 脳卒中の外科 38: 101 ~ 104,2010
