患者は52歳の男性.身長 165cm,体重 45kg.数 年前に原因不明の頚椎骨折を指摘されたが,精査せ ず放置していた.2ヵ月前より背部痛,全身倦怠感, 食欲不振があり,ほぼ臥床で生活していたが,症状 が悪化したため自身にて救急要請し搬送された.救 急隊到着時,意識レベルはJapan Coma Scale II-2R, 血圧 170/120mmHg,脈拍 140/min,呼吸数 42/min, SpO2 82%,体温 37.7°Cであった.病院到着時の身 体所見では,頚静脈が怒張しており,両側全肺野に 湿性ラ音を聴取した.また,肋骨数箇所に多発膨隆 部位を認めた.全身CT検査を行い,右第 2・5肋骨, 左第8肋骨に石灰化を伴う骨肥大,右第11肋骨・ 左第8肋骨骨折,右肩甲骨・胸椎・腰椎・仙骨に骨 融解像を認めた.血液生化学検査では,ALP 2,491 IU/L,BNP 460pg/mLと上昇しており,各種悪性 腫瘍を除外し,骨Paget病,心不全と診断して入院 日臨麻会誌 Vol.34 No.7, 840 〜 844, 2014
