側弯度が100°を超える高度側弯症4例に対し, X線透視下に2例に硬膜外麻酔, 2例に脊髄くも膜下麻酔を行い, 硬膜外腔穿刺2例, くも膜下腔穿刺1例に成功した. くも膜下腔穿刺1例は穿刺距離が深く, 針先が届かなかったため穿刺に失敗した. 透視下で穿刺する際, 棘突起, 両側椎弓根を確認し, これらの目標が最も見やすいような角度で透視をすることが成功の秘訣と思われた. 高度側弯症例での透視下くも膜下腔・硬膜外腔穿刺はかなりの熟練を要するが, 安全に施行できる.
