初診時のX線 写真 を図1に 示す. 主訴である動揺 してい るブ リッジ支台歯の うち,動 揺 度III度 の 歯113を 抜 去 後,即 時 義 歯 (2+35-7)を 装着 した.不 適合歯冠修復物 を撤去 し暫間被覆冠 を装着後,歯 周初期治療 を行 った.そ の 後,歯 周 ポケ ッ ト4mm以 上の部位(6-3,6-4) に歯周外科処置 を施行 した.4は 動揺 が認 め られ な か った ものの,歯 周疾患によ り骨 吸収が生 じていた. そのため,歯 根歯冠比 の改善が必要 と考 え,歯 内処置 を行 い,磁 性 アタ ッチメ ン トを装着 した. 3543を 支台歯形成後,内 冠 を製作 した.内 冠 の 43間 に リーゲル孔 を設定 し,旋 回 リーゲル レバー2) の製作 も行った.そ の後,義 歯 の印象採得時 に内冠 を 取 り込 み,外 冠 および義歯の作業用模型 を製作 した. 垂直 的 な顎位 は安静 空 隙利 用法 によ り求 め,そ し て,Willisの 顔面 計測 法 も参 考 に し,決 定 した後 に 咬合採得 を行 った.通 法 に従い フェイスボウ トランス 日本歯科大学歯学部歯科補綴学第二講座 Department of Crown and Bridge Prosthodontics, The Nippon Dental University School of Dentistry
