近 年,顎 関節症 あるいは顎機 能異常 など と称 される咀 嚼 系機能障害 の患者が増加傾向にあ り,現 代歯科医学に お ける重要課題の一つにあげ られて いる1~8).した が っ て今 日で は,病 態お よび病因の解 明 と,適 切な治療法 の 確 立が急がれてい る.し か しなが ら本症の成因について は,下 顎頭偏位説9~11),筋障害説12,13),神経一筋機構失 調説14~16),精神生理学的因子説17,18),精神病理 学 的 因 子説19,20)などの仮説 があげ られている ことか らも判 るよ うに,そ の病態 は必ず しも一元的ではな く,成 因 もまた 多元的である と思 われ る.し たがって本症 に対す る治療 方針の設定は困難 をきわ めることが多い. 著者 らは,咬 耗現象 によって咬合高径の低 下 を 来 た し,咬 合の不調和 を招 いた とみ られる顎 関節症患者 に対 して,occlusal splintに よる可逆的療法 の後,咬 合の再 構成 を施 して,す こぶ る良好な経過をたどった症例 を経 験 した.そ こで,本 症例 にお ける診断,治 療経過 に顎機 能 および咬合の臨床的検査を加えたのでその概要につ い て報告 する.
