総 義歯 を装着 してい るとその床下粘膜 は咬合圧の影響 を うけ,ま た各種の外来刺激が遮断 され るな どの特殊 な 条件下 にあると考 えられ る.と くに適合不良な総義歯 を 装着 してい る場合,義 歯床 の部分的な圧迫 または摩擦 に よる顎堤の異常な吸収,あ るいは炎症 などを起 こす こと は広 く知 られている.ま た床下粘膜の感覚に も種 々の変 化 をきたす と考え られる.床 下粘膜 の炎症に関する組織 学的1,2,3,4)あるいは組織 化学的5)研 究 は数多 く報告 され てい る.ま た口腔粘膜 の感覚6,7)および無歯顎者の口腔粘 膜の角質化8,9,10)につい ての報告 もあ る.総 義歯床 下粘膜 の感覚については田村11),川 口12),の 総 義歯床 下粘膜の 電気刺激感覚閾に関する報告が あ り,温 度感覚閾 につい ては稲 葉13)が無歯顎者 の床下粘膜 および有歯顎者の 口腔 粘膜の温度感覚閾に関 して報告 しているだけである.し か し同一個人について適合不 良な総義歯を装着 していた 時お よび適合良好な総義歯 をある一定 期間装着 した後の 床下粘 膜における温度 感覚閾値 を測定 し,そ の閾値の変 化についての研究は,著 者 の知 る限 りでは,ま だ行なわ れていない. 著者は田村 ら14)が開発 した 口腔 内温 ・冷刺激装置 を使 用 して,適 合不良な総義歯 を装着 していた時 の床下粘膜 における温度感覚 が,適 合 良好な総義歯 を装着す るこ と によ りどのよ うに変化す るか を追究 したので,そ の結果 を報告 する.
